How Claims Are Paid

Medical Plans (please select your plan below)-

• Allied National
• Independence American Insurance Company, Inc.  EDHP™ 2-50 
• Fidelity Security Life EDHP™ 51+ 
• HMO Carriers: Aetna, Kaiser, Western Health Advantage
• All other carriers to include Aetna, Anthem, Blue Shield,
  HealthNet, Pacificare, United HealthCare

Dental and Vision Plans-

1)  The member will visit the network dentist/optometrist and show the BEN-E-LECT ID card.

2)  The dentist/optometrist will send the billing directly to BEN-E-LECT or the selected PPO Network for processing.

3)  BEN-E-LECT will calculate the claim based on the employer's benefit specifications and send payment to the dentist/optometrist.

Group Term Life-

1) Member will notify BEN-E-LECT of claim and submit necessary documentation.

2) Madison National Life Insurance Co. will process the claim in a timely manner and submit payment to beneficiary.
